消防隊員とトラウマティック・ストレス Firefighters and Traumatic Stress 古賀, 章子 コガ, アキコ 60248408 前田, 正治 マエダ, マサハル 40150817 津田, 彰 ツダ, アキラ 外傷後ストレス障害 非常事態ストレス デブリーフィング Post-Traumatic Stress Disorder (PTSD) Critical Incident Stress (CIS) Debriefing 本研究では,福岡市消防局に所属する消防隊員に対して,日常業務によって生じるストレス反応についての調査を実施した。方法は,(1)郵便法による自記式質問紙調査,(2)個別面接による対面調査の二つを施行した。一次調査では,改訂版出来事インパクト尺度(Impact of EventScale-Revised:IES-R)等を用いた自記式質問紙調査を870名の消防隊員に実施した。そして,二次調査では,一次調査で行ったIES-Rで外傷後ストレス障害(Posttraumatic Stress Disorder:PTSD)高危険群として抽出された44名に対してPTSD臨床診断面接尺度(Clinician Administered PTSD Scale:CAPS)等を用いた半構造化面接調査を実施した。現在診断で完全PTSD(Full PTSD)が3名,部分的PTSD(Partial PTSD)が3名,PTSDではない(Non PTSD)が38名という結果が得られた。イベンツ・チェックリストの結果からは,ほとんどの消防隊員が日常業務において非常事態ストレスに曝露されることが明らかになった。また,その経験について同僚や上司に十分話せなかった場合にストレスを抱え込むという回答が得られた。本調査結果から,消防隊員は日常的に非常事態ストレスに曝されており,こうしたことから生じるメンタルヘルス上の問題に対するサポートシステムを始動する必要があることが示唆された。 In this study, we carried out epidemiological exploration for the firefighters belonging to Fukuoka City Fire Agency. The procedure was consisted of two stage. In first examination, several self-rating questionnaires (IES-R, GHQ, etc.) was used for 870 firefighters, and in second examination, 44 subjects who were selected according to IES-R scores in 1^st examination received semi-structured interview (CAPS). The result was obtained that 3 firefighters were currently diagnosed as full PTSD and 3 partial PTSD by CAPS. Especially, to examine events checklist, most subjects experienced critical incident stress in daily work, and had distress not to tell their colleagues or senior personnel.
